Minimum Immunization Requirements for Entry into School

Documentation of a tuberculosis screening done within the 12 months prior to school entry is required. This documentation must be signed by a physician, nurse practitioner, registered nurse, or local health department official. The documentation must include verification of a negative risk assessment or the documented results of a tuberculin skin test (TST) done in the United States or at a US installation within the prior 12 months. If the documentation indicates a positive TST result, a chest Xray report and a recent symptom assessment are required.
The Request for Entrance and Health Information form must be completed and submitted to the child's school. This form is also available at any Arlington school.
